Update: Winona County residents 65+ may register for the COVID-19 vaccine via the link below
Winona County is delighted to be able to start offering COVID-19 vaccinations to Winona County residents over the age of 65 as stated in the current Minnesota Department of Health guidelines. If you are interested in being vaccinated, please review and complete the online form.

Complete the online form for each person in your home who is age 65+. After completing this form, you will be on a COVID-19 vaccination request list with public health. As vaccine supply permits, staff will randomly select from the list and call or email to make sure you can receive the vaccine and schedule appointments.

Kindly wait for our call rather than calling our office to help keep our precious phone lines open for others. If you receive vaccination elsewhere, please share this information with our scheduling staff when they contact you so we can remove you from this list.

If you know someone who needs help filling out this form because they do not have a smartphone or internet access, assist them with completing this form or have them call 507-457-6375. Staff are available at this number Monday through Friday from 8:00 AM to 4:30 PM.

Completing a vaccination request form does not guarantee an immediate appointment or vaccination. Some individuals may not be able to receive vaccination due to their specific health needs.

Please continue to follow face covering, hand washing, and social distancing recommendations.

Tax Forms are here and are located in the library's vestibule on Johnson Street. We have Federal Forms 1040 & 1040-SR and instructions. We also have Minnesota's M1, M1PR, and CRP (Certificate of Rent Paid).

The Minnesota Income Tax and Property Tax Refund instruction booklets are available for a seven-day checkout.

Note:  The IRS does not allow our tax hub to order anything beyond the main 1040 form and instructions, and the State of Minnesota no longer reproduces print forms for tax hubs.

Federal Tax Forms are available on the IRS Website (www.irs.gov).  Minnesota Income and Property Tax Forms are currently available on the Minnesota Department of Revenue’s Website (http://www.revenue.state.mn.us).

If you do not have access to the Internet, you can order Federal forms by calling 1-800-829-1010.  Minnesota forms can be ordered by calling  1-800-652-9094.


Volunteer Income Tax Assistance (VITA)

Free tax assistance for low-moderate income individuals and families.  Appointments are required. Beginning Monday, January 25th, you can schedule an appointment by calling VITA at 507-474-7202. Please see the VITA handout for more information and the flyer that lists all documentation you should bring to your appointment.
